This has happened to me. I think it's just due to lag, as the chest doesn't open and then more problems arise.

I've done this too... it's because you open the chest before the map loads and everything messes up... I think

Also, this is an known glitch that happens to a lot of players in SkyWars and TeamSkyWars, and it was reported some times in the "Bug Reports" section.
