Overwritten items

This happened to me in matches but this time it was more noticeable (The last matches has duplicated my iron pickaxe with another item, it can't be reverted )

No steps are available since it's sudden, however my knowledge indicates this can happen (i dont know how rare it is) at any moment and can't noticeable unless it's part of a kit, oh this made me disappear my enderpearls in case you were wondering
