Items Disappearing

Server: Team SkyWars, but could be any PvP game.
Time: any time

When I hold an item, for example: a sword, it will disappear. The other items can still be used, but only the swords will disappear. Please fix this, it makes PvP Insanely irritating.

Well there are so many bugs in this update .staff has to do so much of work XD

It could be any item, like blocks or a pickaxe. It sounded a bit confusing so I'm clearing this up.
