Pretty sure it happens if you are falling (reaching damage from being so high) and throw the enderpearl, since you were falling at such a height you will probably die (has happened to me before)

Nope. It was like a 6 block drop. (ruined kingdom map). I would not have taken fall damage frm the height.

That's called inertia.

In real life, if you were able to teleport to somewhere while falling down, you still would not be able stop falling (because of the momentum). So, you would still hit the ground like a bag of potatoes and die.

When ur falling and pearling you mustn't hit a wall but if you do it it counts as fall damage and ur dying
