How to claim the YT rank

I can't get the YT rank it says players with 3 or more warning points cannot claim the YT rank

I got an new account it says the same thing.
Plz Help.

Just send us your IGN, proof that you own a channel with more than 100 subs, and at least one brokenlens video.
Then you're good to go

Hey, as i said earlier, we need a screenshot to prove that you're the owner. Without that, it is not possible for us to do so. To send a screenshotm click the orange upload a file button do do so. However please censor any private information out ie: Real name, phone contact
