Can't claim YT rank w/ correct info

I'm unable to claim the YT with the correct provided information. I put in correctly three different times and after the third time, I was told I can no longer claim it. If you could help me with this, I'd very much be appreciative.

Thank you, I think the warning points I got were from me trying to claim too many times with the "wrong" info.

Yes, I've made multiple videos and streams on the server. I just don't do it as often, but I could always change that.
