They don’t change their chromosomes, that’s not how it works. They just don’t feel like the gender they were assigned at birth are correct or they just feel uncomfortable with it. It’s not a mental illness if it is visible physically. It’s something called a genetic problem where the child has, let’s say, xy chromosomes but is born with the physical appearance of a female.

Also it could just be like tomboys. People who feel more like the opposite gender, act more like the opposite gender, etc.
